Abstract

A clinic case of a 62-year-old patient with a history of 14-year evolution rheumatoid arthritis with empirical treatment, who went to the emergency department for fever associated with abdominal pain and constipation. On physical examination, no fever, no signs of systemic inflammatory response syndrome, vital signs within normal parameters, abdominal palpation, it is found enlarged spleen, confirmed by ultrasound and CT of the abdomen. Hemogram with decreased leukocytes and neutrophils, high rheumatoid factor, bone marrow aspirated with hypercellularity for her age were considered as a diagnosis of Felty's syndrome. This clinic case is presented because of the low incidence of this syndrome in the population since about 1% of patients with rheumatoid arthritis have it, and if an early management is started, the progression of the disease can be avoided.
